I am currently studying HNC Health Care and as part of my course I have to complete a graded unit, this will entail three stages; Planning, development and evaluation. This will be carried out while on placement within a hospital setting, within the planning stage I have to choose a patient and assist them with a nursing activity. I have chosen to follow Roper Logan and Tierney twelve activities of daily living the reason for this is that I find it to be the nursing model that is most effective as each activity has its own importance to the survival of life; also it follows the objectives of my graded unit. After reading through Mrs A’s notes I now feel I have a better understanding of her needs so I have decided to carry out the activity on Wednesday 18th April at breakfast time I will implement a contingency plan that if I am unable to complete the activity then I will at the next available meal time. I will follow appropriate policies and procedures to maintain a save environment in line with the health and Safety at work act (1974) and I will make sure that confidentiality is paramount as this conforms with the data protection act (1998) All aspects of Mrs A’s personal care will be met whilst in hospital this is something that that Mrs A has in place in her home life with the support of a home care team. It focuses on individual behaviour towards a wide range of social and interventions. Health promotion is how health is shown for example to challenge obesity change 4 life is used so people can have a guidance on how to make their lifestyle healthier. It also shows that health promotion is aimed at individuals whose health is at risk from any diseases from lifestyle factors, which lets individuals take control over their health. Stereotyping is when a judgement is made on a group of people because of the actions of a few. Stereotyping happens because of misinformation and other people’s opinions.
The government White Paper (2006) focuses on the need for individualised care and calls for service to be tailored around the needs of the individual and not the service provider. It aims to place the individual in control of their life and promote independence, by providing a more flexible service, with a view to a reaching and fulfilling a healthy old age. In addition, violence, stress and abuse which pose a threat to an individual’s overall well being, must be identified and addressed. The nurses did not view Joe as an individual because they did not give him the opportunity to choose what he wanted to eat, or allow him to get dressed before leaving his room. To adopt a more individualised approach, the nurses should have let him choose what tine to get up in a morning, allowing him to get dressed and further choose what he wanted to eat for breakfast, therefore adhering to The White Paper guidelines.

This essay provides a written account of the holistic assessment used when admitting a patient onto a respiratory ward. A brief outline is also included of the processes involved together with the resources used for collating information. Using the Roper, Logan and Tierney activities of daily living (ADL’s), eating and drinking, has been identified as one goal of nursing care. A short reflection has also been included based on experiences gained on a first clinical placement on the ward. For the benefit of this essay the selected patient will be referred to as Mrs P in order to maintain confidentiality.

There are four key characteristics to consider when evaluating a health care system and its effects on a population; these characteristics include access, coverage, quality, and safety (WHO, 2015). Since the ACA is in full swing, these four characteristics can be assessed and evaluated to help measure the success of the new health care system. Access is who gets into the system and who doesn’t and how (Boex, 2015). Many will argue that the ACA failed to provide greater access for the American people. The ACA did significantly reduce financial barriers to care, which helped increased access to care; but failed to provide the additional resources (primary care providers, transportation, flexible clinic hours, etc.) needed to gain access to care. Therefore, access has not been well addressed and continues to be an area for improvement for the American health care system.
